In my case I've always traced it to a typo somewhere or a cut and paste that didn't quite get everything I meant to or that I pasted into the wrong place. Now I save the menu.xml file after my changes but leave it open in the editor (vim in a terminal window) while I right-click on the desktop to check that everything is working. If I get the little dot instead of the menu I can go back and look for the problem.
