After posting this I had another look... it seems the following PNG's
are not in the "standard" 20x20 size;

23x22 exthyperlink.png
23x22 inthyperlink.png
23x22 remove.png
23x22 kupulibrary.png

16x16 document.png
16x16 folder.png
19x20 superscript.png

And the following gif's that don't have replacement PNG's are also
different sizes;

23x22 kupu_icon.gif
16x16 kupusearch_icon.gif
16x16 zoom-in.gif
16x16 zoom-out.gif

Note: space.gif is a 1x1 "blank", which I don't mention because it
should stay that way.

Should we standardise on 20x20? or given the large amount of empty space
in most of them, should we go for 16x16? I think they start getting too
small at 16x16...
