It turns out that what I thought last Friday was completely wrong.
There is a bug, and it's definitely in Apache, which is triggered by
the following obscure combination of corner cases:

1) SSL renegotiation (initiated by per-directory SSL config)
2) POST requests without Expect: 100-continue (which is essentially
all of them)
3) the standard way of setting up PHP5 under FastCGI (using
mod_actions)
