Well, actually, I have the exact same problem! 

I have: 
Radeon AIW 9000 Pro
Kernel 2.6.15
Xorg 7.1
Direct Rendering is working

The microcode seems to be loaded correctly (same as your xorg.log). However, the
tv-in is still not working.

If i find a solution, I'll let you know -- but i'm working time to time on this
problem for 3 months without any success.

Any suggestions folks?
