add a new configure flag to enable lifecycle logging (which is pretty 
expensive, so it's disabled by default);
add lifecycle logging to ServantBase and RefCountBaseImpl;
fix some ref counting bugs and get the annoying sticky window go away when the 
client finishes (and the exit
button in the upper right corner of the shell window actually works, yay !);
