The device should be created automatically by the Xserver. It may be
helpful to see the full log. The most probable problem is that you don't
have the r128 drm kernel module installed. Try running modprobe r128
from the command line to check that. If you don't have agp support
compiled statically into your kernel you may also have to load the
agpgart module first.
