Can you just throw in inline mml namespaced stuff anywhere a la
xmltex/passivetex or do you need to wrap in an
fo:instream-foreign-object? (just asking to know what is and is not
portable).

Needs to be wrapped in fo:instream-foreign-object. Ditto for SVG.

Epic supports TeX-based math (it uses TeX as the underlying formatting engine).

you mean if you refer to tex math fragments from the fo document?

Not sure--haven't tried this out yet.
