These are really fixed now.
I have to report this is not fully fixed. (I'm beginning to feel like
the Grinch who stole refdb...)
Only 3 out of 4 are 'really fixed': while missing AUTHORLIST, PUBDATE
and TITLE ROLEs are now handled gracefully by refdbd, it still fails if
a PUBTYPE TYPE is omitted. If you omit the TYPE attribute from the
PUBTYPE element -- I know it's forbidden by the DTD but a clueless user
may do it anyway -- and attempt to add the style anyway, refdba/refdbd
hangs.
