Ah!  I have found my issue.  When I first loaded AMP, I had originally tried define some globals in the extensions.conf file and still in fact had a [globals] context in there.  I removed that line and then retried and now it works.  Seems as though the [globals] context was loading from the extensions.conf file, and the [globals] in extensions_additional.conf were being bypassed.
 
Thanks for the help.  I'm sure I'll be asking more questions as time goes on.

If your setting up your PSTN - Inbound phone lines the context should be from-pstn. Not default.  Also if your setting up FXS ports for analog phones or fax machine the context for those is from-internal.
 
The default is there but it's setup for giving a congestion instead of routing. You can change that but it's not a good Idea.
