Comments on your guide compared to my production systems already running
Debian.

I haven't found a need for the MySQL patch. cdr_addon_mysql.so compiles
and runs with libmysqlclient14-dev.

There is a Debian init script in CVS. Look in
asterisk/contrib/init.d/rc.debian.asterisk

A hack I've found is to install Asterisk from Sarge, then 'apt-get
remove asterisk'. It will keep the permission settings, init script and
configs but allow your CVS package to overwrite things. There has also
been talk on various lists of an automated way to build a .deb from
Asterisk CVS.

I have a bug relating to Debian on sf.net:

This will be very difficult. Many components of the AMP source will need
to be changed to comply to the FHS and some licencing issues with
packages need to be taken care of. Particularly those covering LAME and
mpg123.

Add the non-free line to /etc/apt/sources.list and apt-get install
mpg123. My version installed from sarge non-free reports:

pbx1_5:~# mpg123 -v
High Performance MPEG 1.0/2.0/2.5 Audio Player for Layer 1, 2 and 3.
