as you might expect, such things are never simple, and rarely one  
sided, but my understanding was she was:

 she and Tony had very different ideas of the future of BP, and she  
wasn't able to deal with the conflict, so she sent him packing,  
forever persona non grata, at least during her tenure.

not too sure why none of the other members of the board didn't have  
the balls to mtell her where to head in....

a fairly typical autocratic reaction to being opposed.
