I'd like to avoid having mutually exclusive extensions if possible.

Pierre, are your API's all prefixed with cairo_ too?
How close is your extension to the cairo api, is there overlap in your
function names and those from the wrapper?

To make a long story short and as you already said and proposed, there
is no problem to name the package cairo_wrapper and release it under
PECL. As it was always named this way, I do not see an issue. And
there is no need to rename the function just like you never use
"wrapper" in your function names. The "cairo" and your extension will
simply be mutually exclusive.
