Cool, that is much cleaner.  Sorry to make such a mess of your code ;)

I pulled in the CSS earlier, because I see this thing as being an Alias
line in the httpd.conf file and as such it can't really depend on
anything since that would require installing those dependencies in the
document root which most distros want to avoid.

Also need to do something more intelligent and actually ask for the
password on the clear cache and view user entry options.  Right now that
password is just a flag.
