Microsoft had a program called "Anomaly Tracker" that I have used in the past.
Not sure if they are still updating this or what the cost is though.

I've also used a web based system called "eLog". This is a free CGI script.

The VS 2005 Team System sounds very promising
http://lab.msdn.microsoft.com/vs2005/teamsystem/ , but it is only in the
"technology preview" version right now.
I didn't see any info about the price or if you can use it with VS 2003.

See the whitepaper "Visual Studio 2005 Team System: Enterprise-Class Source
Control and Work Item Tracking" [1]

Team System won't be for everyone (TCO, ROI, etc.), but if you might be
using it when it releases later this year, you may want to make only modest
investments in other tools right now.

Vendors of other work item tracking systems may integrate into Team System,
so that might be something to check for when looking at other solutions.
