Is this using the stock kernel and oprofile RPMS from RHEL4 in a RHEL4 
installation (no custom local built kernel or OProfile)? If so, you should file 
a bug report on bugzilla.redhat.com. The bug report should include the 
information about versions of OProfile and kernel, the /proc/cpuinfo, the 
/root/.oprofile/daemonrc, and clear instructions on how to reproduce the 
failure.

Is this a 32-bit or 64-bit kernel? I have a similar machine running a 64-bit 
kernel and RHEL4U4 installation and it seems to be doing okay with the 
2.6.9-42.0.3.ELsmp kernel.
