Ok, I've put some more hours of work into it and tried to fix the most 
obvious errors. What I saw during fixing it was that many things 
actually went wrong while doing the automatic transistion from SGML. For 
example, the reference chapter listed only 'set' commands instead of 
'reset', 'unset' and 'toggle' because they all had the same link target.  
There're likely more of this issues. Also, the index should catch 
everything I tried him to tell to so that it should be okay.

If anyone wants to help: the manual now must be read to

- find things not covered by index although they should be (environment 
variables etc)

- things that don't make sense (e.g. as for the 'set' issue above, it 
could be that there's a reference to the 'lists' command when 'unlists' 
is meant because of the same reason as for 'set')

Also, I sanitized the link targets (e.g. 'command-' prefix for config 
commands, more to come), the init.h magic doesn't work any longer 
because makedoc just catches links and not different types of them. But 
makedoc must be fixed to clean all of this up a little. I need input on 
that so that I can do something on this when I'm back from vacation in 
October. Maybe we move to a custom XML DTD completely and build 
everything else from it as we already have a dependency on XSL 
processors?
