multipath enhancements to md 2.4 + mdadm fixes

Hi Neil,

I've made the mdadm + md multipath changes available at
ftp://ftp.suse.com/pub/people/lmb/md-mp/. I've not attached them all to save
on list bandwidth.

Could you please review and consider merging?

I think that at least the compile fixes for mdadm are worth taking.
