I have not encountered it either, but I read about it on Xboxhacker.
Its some
kind of leaked disc used by MS internally to convert a retail machine to
a
debug machine or somesuch (I didn't follow this news too closely as it
is not
so interesting to me). You are unlikely to meet it in the wild by
accident.

Actually, this recovery disc is used for the debug kits only. What it
does is it updates the debug kit to the latest debug kit software and
ROM. Also, it will turn a debug kit into a retail machine, making 64MB
of the 128MB unusable in this state. It then can be re-inserted to
convert the machine back to debug. That is the only real purpose of it.
