breakpoint state transition in remove_all_break.

In current KGDB CVS code, in remove_all_break states of breakpoints are
set from bp_set to bp_none. It should be from bp_set to bp_removed.

I have also called kgdb_arch_remove_breakpoint instead of kgdb_set_mem
in remove_all_break for handling "Break instruction address alignment on
ia64".

Attached patch does this all.
