The card connects first but then disconnects. I guess you have a scripts
that sets some parameter that requires reset and prevents further
association.
I don't know if it helps, but this is what I get with the module from linux
2.6.6:

eth1: no IPv6 routers present
eth1: New link status: Disconnected (0002)
eth1: New link status: Connected (0001)
That's the same thing except that the card associates after disconnect.
It's better that you set the parameters first and then bring the interface
up. This would prevent the disconnect in both cases.
See ifconfig and iwconfig output for the interface. Try to figure out
what scripts touch the interface after it has been brought up and what
exactly they do.
If you are using WEP, please make sure you use the correct key and the
authentication algorithm.
Any help would be greatly appreciated. Also, feel free to ask me for more
information if it could help, as I wasn't quite sure what else could I sent
to help.
Please post ifconfig and iwconfig output after the association failure.
