this is driven by 'showmode' setting, that is of in compatible mode by
default.
please verify you are launching 'vim' (and not 'vi' that is in
compatible mode by default).

Are you running vim as 'vi' or as 'vim'? I guess the former. In this
case the behaviour you are reporting is the expected one, since
'-- INSERT --' is shown only when vim is not incompatible mode. See the
latest NEWS item in /usr/share/doc/vim/NEWS.Debian.gz
