GSL documentation with PDF 'index'

Hi all

Does anyone have a copy of the GSL manual (PDF) that includes clickable
'index' headings and cross-references? This would make the PDF manual
much more easily navigated when hunting for a particular chapter or
section on-screen.

For what it's worth the GiNaC people seem to have worked out how to do it...
