In addition to the good stuff you've said, I believe that it stems from 
a bug in XML::SAX's Makefile.PL: if you don't have that file it should 
never ask whether or not you want to update it. If you say no, I believe 
it never creates a file, and then naturally proceeds to happily not work 
at all :)
