Comment:

 1.  Yes, it's per module.  I've added a note to that effect in the user
 manual

 2.  Yes, it's a feature.  The user manual describes it in detail in the
 [http://www.haskell.org/ghc/dist/current/docs/users_guide/type-
 extensions.html#instance-overlap section on overlapping instances]

 3. This is a palpable bug, thank you.  I have just fixed it.
