* Remove the use of <multimap>, which wasn't compatible with FD
  and only necessitated a one-line change.
* Fix entities-definer macro to work in FD.  I THINK this change
  should be compatible with GD but I haven't tested it yet.  Any
  volunteers?  If not, I'll have to use gd/fd-specific .lid files.
* Create <node-mixin> in xml-parser module instead of in interface.
* Remove a lot of warnings...mostly unused definitions and import
  ambiguities for format-out.
