Testing again with kmail/KDE 4.3 RC3 I see no problem with all mentioned steps.
No messages, no display problems. Seems to be fixed already.

The easiest to click options are "Ignore" and "Add to Dictionary", which are also the only undoable options. Although in a spellcheck dialog these might be the most common options, in this use case users are unlikely to right-click a word which is already correctly spelled. 2. Actually correcting the word requires two clicks, which is unnecessary as the first menu is not excessively long.
