Try these settings out:
<inputDocument documentFormat="xml"/>
<documentClass dom="xhtml" generate="both"/>

Lazydom I don't think can handle XHTML.

Also make sure your outputOptions have omitXMLHeader = true,
omitDocType = false,
enableXHTMLCompatibility = true, useAposEntity = false, mimeType = text/html
