Need website developer proficient in forms/databases

I need to hire a web developer URGENTLY for the production of a single
page form with linked database to add to our website. I would like help
with the overall website design as well later, but the urgency is for a
single page and database.  You can see our website at
www.drpaul4smiles.com.  We are running a contest as part of the Women's
Expo in Phoenix and need to collect basic data such as name, address,
phone and email as contestants place their entries online.  This first
part should be a quick and easy job for the person with the right
knowledge.  Our database and website are hosted by godaddy.com.  Please
