comments on CMMF

A few comments on CMMF:

- I wonder if CertRep definition should not be changed to have rspInfo
field optional. The current definition is:

CertRep  ::= SEQUENCE {
    response    SignedData, OPTIONAL
    rspInfo     OCTET STRING }

As far as I understand rspInfo corresponds to regInfo, and since regInfo is
optional shouldn't rspInfo be optional as well? If it is not optional, what
should  it be populated with? The text following the definition states that
it MAY include state information etc. A MAY is not a MUST, so what happens
in the case it is not to be populated? 

- Section 2 - abstract identifies ietf-pkix-request@xxxxxxxxxx as the
address of PKIX mailing list. This is no longer true and a link to
ietf-pkix-request@xxxxxxx should be stated instead.

- Throughout the document there are errors of the type "'" replaced with
"M-F".

- Section 5.9 - the first sentence starts with ">". The second sentence
contains double word "and".

- The references section contains an old reference to CMS draft.
