You may have a look at: 
http://www.gpsbabel.org/htmldoc-1.3.3/Styles.html, as you could write a 
style file - basically a description of a specific csv format - to 
convert your data.

As I'm a gpsbabel newbie myself, I don't know if this will suit your 
needs in your special case - but doesn't look bad at first sight.
