We were given something that had a difficulty and a terrain. We
weren't given (or couldn't recognize) a cache type, so we picked
unknown/mystery.

Correct. If you specify a deficon, that will be used in preference to
the geocaching.com icon set when we are processing geocaches.  (How do
we know we're processing geocaches?  We have a difficulty and a terrain
that are not zero.)
