HEAD / opensc-0.9 changes

Hi,

I merged bug fixes from head to opensc 0.9 and vice versa,
and merged my fixes for windows/cryptoflex/egate and the
new improved profile code.

There is one big change for all users of HEAD and opensc-0.9
branch: the svn version now requires a configfile opensc.conf
and will not work without. Under *ix confiure the configfile
(--sysconfdir=) location and put the current file there.
Under windows add a registry key HKLM\Software\OpenSC\ConfigFile
with the path to the config file in it.

You can also use an environment variable OPENSC_CONFIG under *ix,
or the HKCU\Software\OpenSC\ConfigFile registry key under windows
to set the config file.

If you want to use your current config file, please add this section:
        # PKCS#15 initialization / personalization
        # profiles directory for pkcs15-init.
        
        profile_dir = PKGDATADIR;

and replace PKGDATADIR with the directory that has your *.profile
files. ($prefix/share/opensc/ usualy.) The code will now no longer
search in the current directory for profile files for security
reasons. Also the code no longer uses the access() function that
causes problems with setuid applications.
