Citation Verification: American Journal of Pain Management

I need the following citation completed and/or verified.  I have been
unable to locate this article on the AJPM web site and am wondering if the
scanty info. I have is correct, and this journal doesn't seem to be indexed
in PubMed.

Moskowitz M.  Pathogenesis and treatment of chronic back pain in the
elderly.  American Journal of Pain Management 14(?):??-??, 2004.

As you can see there are a lot of ??s in there!
