When we were using Serials Solutions, Chris suggested putting
all the host entries in a separate file, hosts.cfg, which is
referenced by ezproxy.cfg.

Initially, we extracted all the hosts from the SS feed, sorted
and de-duped, and created hosts.cfg from that. Someone with
more of a programming bent could make this an automated task.
However, after a settling in period, there are relatively few
additions, which we handle on an as-reported basis. There is
still enough "art" in setting things up that we prefer to
maintain hosts.cfg manually.

We also use a monitoring script that sends a note to EZproxy
admins when a new host entry is required. This only happens
once or twice a week, so dealing with it manually works fine.
There is probably some dead wood in hosts.cfg, but it doesn't
seem to be causing problems.
