Ed's suggestion of placing such database definitions before any other AutoLoginIP/ExcludeIP lines is indeed the recommended way to handle forcing the proxying of select databases. The other factor to consider is that if the resource still recognizes all of your local IP addresses, people could still skip around EZproxy and get to the resource, so you may have to make additional changes beyond EZproxy if you need to enforce this.
