I think we might want to distinguish between rating general proficiency in speaking English and the degree to which students have learned to talk about the things that are taught and practice in a course. For example, if you teach students health-related vocabulary, have them do role plays in a hospital and doctor's office, give them listening exercises which focus on health issues, etc., then you can test them on their ability to handle those specific topics in the ways that they have practiced in class. This is a bit different than judging their overall fluency in areas of English that they may or may not have been practicing in class.
