File->New
Insert->Floats->Figure
The title of your caption 
Insert->Math->Inline formula
Y=a+bx<space>
 contains a math inset.<return>
Insert->Graphics
(Insert the name of some graphic file and press OK)
Layout->Paragraph
(Set the Alignment parameter to Center and press OK)

Voila. You have created what I sent to you.

Read the User Guide. It was written for a purpose you know ;-)
