Well, that is what is to be expected.

> Preface (second and third pages) -- numbered 4 and 5

I just played around a bit. As \frontmatter sets the pagestyle accordingly,
the following should work (i.e. works for me):

<title line>
\thispagestyle{empty} % needed for page 1
\pagestyle{empty}
<pretext>
\frontmatter
<toc>
\mainmatter
<main text>

Does this fix the remaining problems?
