I've just changed the error messages in the CVS version to print the
command number for failed commands.  You may want to try the CVS version:

Thanks for the reply. I'll get a message out to you either very late tonight or mid-day tomorrow (EDT). I was using a packet sniffing tool when I got the errors, I think airsnort or a program along those lines (and no, i have no interest in "cracking" networks or anything, I'm just a sysadmin/*nix analyst and was looking at our wep setup here). I'll post the outputs of everything as soon as I can reproduce the error(s). I'll also shoot you the iwconfig file after the card fails.
