Summary: A patch mostly for ct-lib

Initial Comment:
This patch contains a number of items.
The largest of these is a reworking of the ctlibrary 
results processing.
The next most significant is a first implementation of the 
wire protocols for server side cursors, and the ctlibrary 
functions necessary to interact with them.
I've also done some work on the bcp de-dblibbing
A few other minor fixes.


My patch directory contains a diff -u output with the 
snapshot of 30th October, and after images of all the 
affected files.
