No Route To Host Issue

Hey Guys,
 
I hope someone out there has seen this issue before. I am running
freeTDS-0.61.2 with Apache-2.0.47, PHP-4.3.2, on a RH9 server. I am
trying to do a simple test connection to the Sybase server listed in the
freetds.conf. It appears to be having some kind of issue with the IP
address.
 
Here is what is in the /tmp/freetds.log:

This is what I get in the Apache error_log:
src/tds/login.c: tds_connect: 192.138.151.39:4444: No route to host
 
After many hours of looking on the message boards I haven't come across
anyone else having this issue. Am I missing something simple here?
