Subject: ODBC critter soup patch

Since error types went away.. got rid of state2/3 and consolidated
the native mappings in odbc with the big one in TDS.

SQLDriverConnect causing 'invalid string or buffer length' /w
the Microsoft DM.

Updated build environment for MSVC. In this diff renamed replacements/
iconv.c to iconv_r.c to get it to compile without clashing with iconv's
object file.

Fixed a memory leak
