Help

Please help with the attached code. The user input validation is not work. it 
suppose to display or the form recall it self with a message when a user misses 
one of the required fields. Also, most important is that on the NEWSLETTERS 
column, when I select more that one item, the procedure inserts only one item 
into the database.

please your help is need. The last problem is how can I make the application to 
retain an already input information without resetting it is if the user makes a 
mistake and the form recalls itself with a message, so that the user don't have 
to re-enter the already correct infors, but only the missing field/s
