I can sit for hours in recumbent position and never feel any strain in my neck. I try sometimes to use the headrest on my velo, but it is only useful when standing still. While riding the "rest" will knock my head around, not comfortable at all. It is an usual comment from people that they think the recumbent position looks uncomfortable because of the bent neck. If you are trained to sit very straight in a chair, I do think it can be a problem, because you try to have a straight neck even in recumbent position.
