I have a book called Literary Theory: A Practical Introduction that does what
you want--I think. It applies the standard range of theories to a selection of
literary texts. It's from Blackwell and it's meant to align with my
anthology--Literary Theory: An Anthology (with Julie Rivkin). I wrote the book
as a simple, accessible intro to the field. Mostly applications, some
explanations.
I'm just now finishing a second edition that incorporates film that will be
twice as big as the first edition and much more comprehensive. If you have
requests for specific needs you'd like to see addressed, let me know. I'll be
finishing it up this coming week. Feedback always helps with projects like
this.
If you're looking for something more in CS, you might look at Susie O'Brien and
Imre Szeman's Popular Culture. Blackwell also has a book called Cultural
Theory: A Practical Introduction (forget the authors) that might be a good
companion, but it's less theory and application than just "how to do cultural
studies."

Foss' (1996) Rhetorical Criticism: Exploration and Practice (2nd ed).
Waveland Press. Each chapter surveys a particular rhetorical approach
(neo-aristotelian, cluster, fantasy-theme, feminist, pentad, genre etc.)
followed by sample essays which use that approach. The sample essays are
both reprinted essays from rhetorical scholars and student papers from
past rhet theory classes.
