Check to see if all the email addresses on the list are valid addresses. As
in they subscribe to name-omLZJFyqO6s4Q++5jOxPmw@xxxxxxxxxxxxxxxx convention.
I had a similar problem. Got fixed after I noticed one of the email
addresses was just a name.

Just guessing -
If you manually loaded up the psl_mailinglist table, did you increment
the id field so they have unique id's?
Also the value of psl_mailinglist_seq in db_sequence should be the max
psl_mailinglist id.
