This is actually a problem: Most building compoonents do not repeat in
a 100x100 grid. It means complicated maths to get the repeats to work.
For example standard UK bricks are 22.5x7.5 nominal (i.e.including a
joint), so stretcher-bond walls have an effective repeat 22.5x15 (in
which you see one whole brick and two half-bricks above it). Other
countries have other standards. Paviors and tiles have yet other sizes.

A really workable pattern system would have x and y defined (for example
by a diagonal or a rectangle on a non-printing layer)

And, as I've mentioned before, it would be great to be able to scale all
insertions as well as patterns independently in object x and y axes.

I have had the same problem, and had to resort to placing a dot at the
top right corner of the intended repeat (in pen 0, so it barely shows).
I haven't done this recently, though so may be out of date for current
version.
