cant get to log and pass

hi for the life of me i cant work out why im unable to simply log an threat and pass the request though, for one alert its a urlencoding issue and it keeps going to a 403, i tried to put a status of 200 and just comes up as ok, im going to have to turn it off for now

SecFilterDefaultAction "auditlog,pass,status:200"

we've been getting some exploits in php allowing files to somehow magically appear, i might need to know soon so i can trap whenever it happens again.
